2009-2010 Player Registration

*** Note on confirmation of registration:  You will receive a confirmation number and a confirmation email from treasurer@lakevillehockey.org.  If you do not receive the confirmation number and email, you did not successfully register your child.  Please contact treasurer@lakevillehockey.org if you do not receive the confirmation email.

Here are some key points about this year's registration process:

1.      Online registration.  All registration will be done online.  Registration will start on August 7, 2009.  Registration will conclude on August 17, 2009 for Squirt, Pee Wee, Bantam, U10 and U12 levels.  Registration will reopen on September 11, 2009 and conclude on September 21, 2009 for Mite Prep, Mite and U8 levels.  A late penalty of $25 will be assessed if registered after the closing date.

This year, USA Hockey registration will need to be done separately, and must be done prior to LHA Registration.  This $40 fee ($30 USA Hockey, $10 Minnesota Hockey) is not included in LHA registration fees this year.  Please see the following user guide, which details how to proceed with USA Hockey registration before continuing on to LHA Registration.
  USA Hockey/LHA Registration Help Guide

2.      Birth Certificate.  A copy of your child's Government Issued Birth Certificate (not a hospital or baptismal certificate) will be collected by your team manager once teams are formed.  Your child will not be allowed to play without a birth certificate on file, due to District 8 and USA Hockey rules.

3.      Payment options.  In order to complete registration, electronic payment of registration fees must be accomplished.  Payments will be accepted at time of registration with Visa/MasterCard credit and debit cards via our online registration system.  In an effort to streamline the payment process and eliminate collection issues, all payments will be electronically guaranteed.  This will eliminate the need for you to mail in your payment to LHA.

4.      Cost.  The online registration fee is made up of the following costs:

  • Equipment costs
  • Use of home and away jerseys for all traveling teams
  • Jerseys to keep for all Mite Prep, Mite and U8 teams
  • Evaluations
  • Administrative Costs
  • Fundraiser fees are not included in registration costs
  • USA Hockey and Minnesota Hockey fees are not included in registration costs
  • Warm-up attire is not inlcuded in any LHA costs this year.  First year U10/Squirts, new LHA players, and players in need of a new jacket will need to pay for their LHA jacket through Westwood Sports at the warm-up sizing dates.  Further warm-up info will be posted on the LHA website.

5.      Registration fees.  The Projected Additional Fees are estimated and may vary depending on individual team budgets, players per team and ice costs.  All Mite Prep total season fees are paid at registration.  The $100 fundraiser fee is included in Projected Additional Fees for all levels except Jr. Gold.  The fundraising cost can be recovered by selling the raffle tickets.  Please refer to the LHA Handbook for more detailed information on estimated costs.

The Project Additional Fees will be due to LHA or your team manager depending on which expenses you are paying.  The fundraisers will be handed out at tryouts/evaluations and will need to be picked up before the player is allowed to take the ice.  The bulk of your remaining hockey costs (ice time, tournaments, fundraiser) will be due and paid to LHA on November 15, 2009 with the final payment due by January 15, 2010.  Once the teams are formed, your payment will be adjusted for that team's actual costs.  If sponsorship money is received this year, a credit per player will be applied toward your team payment, based on the total amount of sponsorship income.

Final adjustments will be made in March 2010 based on actual costs incurred for your team.  March adjustments must be paid by March 31, 2010 Each team member will pay either LHA or the team for ice, district fees and other costs incurred by LHA.  Miscellaneous expenses (i.e., referees for scrimmages, pizza, coach’s gifts, etc.) will be collected by your team manager and held in an account by your team manager in a non-LHA checking account.

6.      Arena Fee.  In 1994, the LHA Board of Directors implemented an “Arena Fee” to raise funds upon its inception for an arena.  A yearly “Arena Fee” of $35 per family is collected from all families that have not yet contributed a total of $245.  This fee will be assessed each year until the long-term debt is retired in 2017 and the arena fully covers its expenses.  Families are welcome to contribute $245 to the arena fund, now or in the future, and subsequently have the $35 Arena Fee waived for the balance of their years in the LHA.

7.      Fundraiser.  For the 2009-2010 hockey season, the fundraiser will be handed out at tryouts.  The cost will be $100 for the first child and second child.  There will be no additional charge for families with more than two children playing for LHA.  No player will be allowed to tryout or be evaluated without picking up their fundraiser packet.

At tryouts/evaluations, each family will be given an envelope with (5) raffle tickets to sell for each participating child (capped at 2).  Each raffle ticket will sell for $20 and have a chance to win cash prizes.  You will be required to return the filled out portion of the raffle ticket to your team manager later in the season (date TBD), along with the $100.  If you elect to “buyout” rather than “sell” your raffle tickets, simply return them to your team manager with your name filled in on each stub along with the $100.

8.      Tryouts.  Tryout schedules are posted on the LHA web site. All tryout fees are included in your registration fee.  We recommend that all players participate in evaluations.  The evaluation process allows the LHA to create evenly competitive teams for the benefit of every player.  A player/member is allowed to tryout for only one level per season.  The level in which a player tries out for, is the level they will be assigned for the regular season.

9.      Policy on High School Tryouts.  If your son or daughter elects to try out for a high school team, please be aware that the registration fees cannot be refunded to you.  The registration fees are for insurance, equipment, jerseys and other costs needed to operate the LHA.  You will also be responsible for covering your share of the team costs incurred up to the time you leave the LHA and move to the high school team.

10.   Concession Stand Volunteering and Mandatory Volunteer Hours.  The LHA operates the concession stand at Ames Arena, with the profits earmarked for special projects, such as covering the cost of ice scheduling and new food equipment.  We depend on each team to volunteer 4 days during the hockey season to operate the stand.  If your team does not work its appointed shifts, the team will be charged a $75 fee which would represent lost profits while the stand is closed.  NO CHILDREN UNDER THE AGE OF 16 WILL BE ALLOWED IN THE CONCESSION STAND AT ANY TIME.

This year, LHA has instituted a mandatory volunteer program.  Each family will be required to volunteer for one 4-5 hour shift.  A variety of volunteer opportunities will be posted on the web site.  Each family will be required to complete their assignment before the end of the 2009/10 hockey season.  There will be a $125 fee assessed with your last team payment for any family that has not met their volunteer commitment.

11.   Parent Meeting.  There will be two different parent meetings.  The first is for all traveling levels (Squirt, Pee Wee, Bantam, Jr Gold, U10, U12, U14), and will be held on Tuesday, September 22nd, at 7:00 pm at the Lakeville South High School Auditorium.  The second is for Mite Prep, Mites and U8 only, and the date is to be determined (sometime in October).  These meetings are intended to provide our parent members with the most current happenings at the LHA.  This is an opportunity for you to meet your board members, catch up on activities from over the summer, and ask your questions.

12.  First Year Equipment Credit.  The Lakeville Community Corporation has generously donated upwards of $37,000 to help defray the cost of hockey for brand new members of the LHA.  First year kids new to the LHA registered at the Mite Prep through Mite 2 levels will be given a coupon at the Mite/Mite Prep Parent Meeting that will entitle them to a one-time credit up to $50 on hockey equipment purchases made at the stores listed on the couponNote that only equipment (not sticks, skate sharpening, etc) is eligible for the coupon.  We are proud to take part in this terrific program.  Thanks to the Lakeville Community Corporation for their support and generosity!

13.  Scholarships.  The LHA has set aside a limited amount of funds for families that are experiencing a long-term financial hardship.  If you feel that you qualify, please contact Kent Johnson at 952-457-2723 or treasurer@lakevillehockey.org.  All Scholarship requests are held in strict confidence.

14.  Refund Policy.  Beginning with the 1995-96 season, the LHA adopted a formal refund policy that will be applied in all cases when a registered participant wishes to withdraw from the program.  This policy was adopted with consideration of the fact that the LHA incurs costs immediately after a child is registered.  Many of these costs are not refundable to the LHA by the District of USA Hockey, and as such we cannot refund them to the participant.  If a withdrawal request is made prior to the actual tryouts, you will receive a refund of the registration fees, less a charge of $50 to cover administrative costs.  If your child attends a tryout session, there will be NO refund of any registration fees.  Injuries, although rare, are part of the game of hockey, and as such, are not allowable reasons for withdrawing from the program and requesting a refund.  If your child is injured during the year, they are responsible for all team payments during the time of their injury.

15.  Contact information for questions.  For questions regarding payments or costs, please contact LHA Treasurer Kent Johnson at 952-457-2723 or treasurer@lakevillehockey.org.  For questions regarding registration, please contact LHA Registrar Linda Freemark at 952-985-5716 or registrar@lakevillehockey.org.  For all other questions, please contact the appropriate Level Director or Board Member (contact info located on website Board Members page).

When you complete the registration process, an e-mail will be sent to you confirming your entered information and links to an LHA Handout and a MN Hockey Consent to Treat form.  Please use the LHA Handout for any future reference needs, and print, complete and return the Consent to Treat form at the parent meeting.

*** Note:  Please have your Visa/MasterCard and card security code (3 digit code on the back of the card) available when proceeding with the registration process.  You will not be able to complete registration without the correct 3 digit security code.  Please do not register until you have this code.

*** Note:  The address you enter must match your credit card billing address.  LHA Registration includes a verification process that will reject the card if it does not match.
